How to Request a FMX Account

Step 1: Request an FMX account by emailing facilityrental@hempfieldsd.org with the following information:

  • Organization information including: name, physical address, email address and phone number
  • Insurance certificate 

    - Certificates of Liability must name Hempfield School District as the certificate holder. Please use the following address when requesting the certificate from your insurance.

  • Nonprofit organizations must submit a 501c3 form.
